Adding Facility/Residential Placement Programs

Using the Program Placements for Facility/Residential #

Intake #

Once you have selected or created the client account, choose “facility” followed by “facility placements.”

Select “Start a New Intake.”

Fill out any necessary intake details and click save.

Field descriptions part 1: #
  • Facility Placement number: if box is unchecked, a number will be auto-filled. If checked, enter your own number.
  • Intake Date: will autofill when you fill out the intake. Date and time can be edited.
  • Duration/Estimated Release Date: these two fields are connected to each other. If one is filled, the other will autofil.
  • Reason for Intake: Modifiable dropdown menu from the picklist admin ‘facility placement intake reason type’
  • Delivered by: Text box that can be filled in.
  • Delivered by Title: Modifiable dropdown menu from the picklist admin ‘delievered by title type’
  • Delivered by Signature: Esignature that can be used with a touch screen or touch pad.
  • Intake staff: Dropdown menu of staff in the system
  • Intake Supervisor: Dropdown menu of supervisors in the system
  • Intake Authorized by: Dropdown menu of staff in the system
  • Checkboxes:
    • Police Report Received
    • Is Prelim needed
    • Was contact made?
Field Descriptions Part 2: #
  • Facility room: Modifiable drop-down from admin picklist ‘facility location’
  • Facility Placement Program: Modifiable drop-down from admin picklist ‘facility placement program”
  • Program Placements: Modifiable drop-down from admin picklist ‘program’
  • Service Plan Type: Optional service plan decider
  • Privelege Level: Modifiable drop-down from admin picklist ‘privelege level’
  • Charges from County
  • Charges Detained on: Modifiable drop-down from admin picklist ‘charges’
  • Charge Notes
  • Checklist (modifiable options from admin picklist ‘checklist’)
  • Associated Objects
  • Tags/Flags

Upon completion, a facility placement intake summary page will be created. An attachment can be added as needed.

Tags can also be printed by the 2nd image in the upper right-hand corner.

On the client’s facility page, there will be a summary box that shares brief details such as initial intake date, dates in detention, units, and estimated release dates (if added).

Release #

To release a client, select the green “release” button on the facility page.

Fill out any necessary intake details and click save.

Once the release details are completed the facility placements will look like something below:

  • Released to Address: Text box to fill in
  • Release Staff: Dropdown menu of staff in the system
  • Release Supervisor: Dropdown menu of supervisors in the system
  • Released by Signature: Esignature that can be used with a touch screen or touch pad.
  • Release authorized by: Dropdown menu of staff in the system
  • Release notes: Notes associated with the release
  • Checklist: modifiable options from admin picklist ‘checklist’

Field Descriptions Part 2: #

  • Services provided: Modifiable dropdown from picklist admin ‘service provided’
  • Remaining Needs: Modifiable dropdown from picklist admin ‘remaining need’
  • Release notes: Any notes associated with the service plan
  • Change client status
  • Status options
  • Tags/Flags

Picklist Setup #

If a user has administrative access, then updating the drop downs and checklists mentioned above are possible by going to the administration tab, followed by picklist admin.

Using the search bar and selecting the field type, you will be able to edit or create new options.
